Access to Get Well Family Health Team (GWFHT) Services
At SimpliCare Medical Clinics, eligible patients may be referred to the Get Well Family Health Team (GWFHT) for additional publicly funded healthcare services that complement the care provided by your family physician.
Through this partnership, patients may have access to:
- Chronic Disease Management
- Health Education & Wellness Programs
- Nutrition and Healthy Lifestyle Support
- Preventive Health Programs
- Other Interprofessional Health Team Services
These services are provided by Get Well Family Health Team and are available by referral from your SimpliCare physician, when appropriate.
