How long are the sessions?
Group education sessions are typically 30 minutes in length.
Can I attend more than once?
Yes. Diabetes education is an ongoing process, and follow-up sessions may be recommended depending on your needs and treatment goals.
Can family members attend?
Family members or caregivers may be able to attend certain sessions. Please ask our team when booking.
Do I need a referral?
A physician referral may be required depending on your situation. Contact us and we’ll help determine what is needed.
What should I bring?
Please bring:
- Your health card
- Current medication list
- Blood sugar records (if available)
- Questions you’d like answered

Physician Referral
Participation in the Certified Diabetes Education Program may require a referral from your family physician or healthcare provider.
If you are already a SimpliCare patient, simply speak with your physician to determine whether this program is right for you.
